Conditions of Gift Award LetterThe letter outlined below, written on company letterhead, is necessary for all gifts from industry and corporate foundations and should be submitted to Gift Administration with accompanying correspondence and documentation.
Conditions of Gift Award Letter
Dear [insert name of faculty member/PI]:
This award is provided as an unrestricted gift in the amount of $[insert amount] to (please specify) The Regents of the University of California, San Francisco (UCSF) or The University of California San Francisco Foundation (UCSF Foundation) from [Insert Name of Entity]. (This section may be modified to indicate that it is a restricted gift to support the work of a specific P.I., and may also include the general area of study to be supported.)
[Insert Name of Entity] may include information on this award in its periodic public reports. References to the award in media releases and/or other use of the name of any UCSF faculty member, employee, or student of UCSF, or any trademark, service mark, trade name, or symbol of UCSF must be approved in advance by the UCSF Office of Public Affairs.
The acceptance of this gift is based on these mutual understandings:
1. All intellectual property or data resulting from the use of this gift will be retained by UCSF. Except as provided in this Conditions of Gift Award, no agreements have been or will be made as to the disposition of intellectual property or data ownership.
2. UCSF will provide acknowledgment of [Company Name] in any publications that may result from the research funded by this gift.
3. A fee of 4% will be applied to this award to cover UCSF administrative and infrastructure costs; an additional 1% will be charged as overhead as the award is spent.
This letter represents the entire agreement and understanding between UCSF and [Name of Entity] with respect to its subject matter and supersedes any prior and/or contemporaneous discussions, representations, or agreements, whether written or oral, of the parties regarding this subject matter.
